What is Anastomo
and how does it work?

A data interoperability platform: records move securely between providers, anchor to a verified patient, and arrive where they're needed — including your EMR.

How a record moves

Two providers, each holding their own piece. One pass through the platform, and both hold the whole picture.

Provider A only A's records Provider B only B's records Anastomo entity resolution identity verification jurisdiction policy consent audit the conduit — never the owner Provider A A's + B's records ✓ Provider B B's + A's records ✓ Patient A's + B's records ✓

Same two providers on both sides — the difference is what they know after the exchange. And the patient's record fills in automatically, every time.

How records find their patient

Verifiable + probabilistic association — never a silent merge.

Lab Result Jen Smith · 1985-03-14 Jennifer Smith · 1985-03-14 health card № 1234-567-890 verified ✓ Rx History health card № 1234-567-890 Entity Resolution verified anchor probabilistic feature association ambiguity → human review Jennifer Smith ✓ Lab Result Rx History every provider's piece one full, rich record

The verified identity is the anchor; the lab result and Rx history associate to it by shared features — and the output is the whole story, not fragments.

What that solves

For both sides of every exchange.

For providers

  • Complete context at the point of care. Records from every network provider your patient has visited — walk-ins, pharmacies, labs, physio, dental.
  • Stop chasing faxes. Receiving through the network is free. Send from the tools you already have: fax, email, API, HL7, FHIR.
  • The PHIPA paperwork, templated. HINP services agreement, privacy impact assessment checklist, breach protocol — we hand them to you.
  • Your own data, cleaner. The network's patient-anchored matching flags likely duplicates in your local system.

For patients

  • Verify once. Your verified identity is the anchor that links your records as they move between your providers.
  • Your whole story, in one place — free. Every provider that joins makes your record more complete. You are never charged.
  • You choose who sees what. Consent directives let you restrict any record type from any provider — and every disclosure is logged where you can see it.
  • Share your records, or steward someone else's. Delegate access lets you manage consent and sharing for a child, parent, or anyone who's named you their steward.

Share data securely

Purpose-bound, minimum-necessary exchange — every disclosure evaluated against policy, filtered through patient consent, audit-logged.

Coverage that compounds

Inter-provider, across networks, and — as the platform grows — across borders. Every provider that connects makes every record more complete.

Every format, ground up

HL7 v2, FHIR, structured APIs, even scanned text — one typed interchange layer for the formats health data actually lives in.

AI-native, MCP built-in

Agents keep records current, route shares to the right recipients, and operate under fine-grained, instantly revocable scopes.

Filing has never been easier

Send records directly from email to your EMR: an agent picks it up, the platform parses and matches it, and it lands in your system under your recorded authorization.

Your data store, improved

Patient-anchored matching identifies likely duplicates and data-quality problems in your own system — connecting makes your records better.

A platform that knows the law of every jurisdiction it touches

Health data is jurisdiction-locked: what a provider may share, with whom, on what legal basis, and in what timeframe differs in every province. Anastomo encodes each jurisdiction's rules as executable policy — all thirteen provinces and territories, the federal layer, and Indigenous data-sovereignty frameworks — and every exchange is evaluated against the policy of the jurisdiction it happens in. Not a compliance claim: a compliance mechanism.

How do we make money?

Providers pay a service fee for the interoperability platform — the routing, matching, and audit infrastructure. Beyond that, we broker licensing deals between patients and requesting organizations, always with the patient's consent — sick-note evidence for an employer, a research study, population-health statistics. Patients never pay: the patient decides, is compensated for it, and can revoke access at any time.

Why trust Anastomo?

The platform is legally a conduit: it facilitates disclosure between the providers in a patient's circle of care and makes its own use of nothing. Every submission, link, and disclosure is audit-logged and visible to the patient it concerns. Full-history access requires the patient in the loop; consent directives are honoured everywhere; and identity is never assumed — records link only to a patient who verified themselves.
